To begin using the Ossur Academy's Medical Justification Aid, you will need to enter both your Ossur "Customer ID" number and your "Ship to ID" number. Your six digit Customer ID number is located on the upper left of your Ossur packing slip. The six digit Ship to ID is located on the upper right of your Ossur packing slip.
